ABSTRACT

Palisaded neutrophilic and granulomatous dermatitis (PNGD) is an uncommon skin eruption and characterized histopathologically by the presence of granulomatous inflammation with or without leukocytoclastic vasculitis. PNGD is known to be associated with various immune-mediated connective tissue diseases such as rheumatoid arthritis and lupus erythematosus. However, to our knowledge, a case of PNGD in a patient with Behçet’s disease is extremely rare and only one case has been reported in foreign literature to date. Herein, we report an unusual case of a 60-year-old female with Behçet’s disease who presented multiple erythematous to flesh-colored papules on the extremities, buttocks, and ear lobes and was diagnosed with PNGD. After the treatment of systemic corticosteroids, colchicine and azathioprine, the skin lesions and oral ulcers improved. The patient is under observation without recurrence of skin lesions for 6 months.

ABSTRACT

Systemic anaplastic large cell lymphoma (ALCL) is an aggressive CD30-positive non-Hodgkin lymphoma. The most common clinical manifestations of systemic ALCL are solitary or multiple large nodular tumoral lesions confined to one body compartment. A sporotrichoid pattern, temporally occurring distally to proximally, is extremely rare. We herein describe a case of CD30-positive, ALK-negative systemic ALCL in a 68-year-old man with multiple nodules and tumors that exhibited a linear distribution and sporotrichoid pattern.

ABSTRACT

Background@#Mobile applications are useful in the field of dermatology as diagnosis and treatment are based on visual inspection. However, in Korea, dermatology-related applications have not been developed effectively despite the high rate of mobile internet usage. In addition, there is no report on the current trends of dermatology-related mobile application. @*Objective@#This study aimed to identify and categorize the available dermatology-related mobile application and compare the current trends in Korea with those in the US. @*Methods@#Eight dermatologic terms were queried in two mobile platforms, Android and iOS. Applications were categorized by function based on their description. The number of reviews, rating, provider, target audience, and price of applications were recorded. @*Results@#A total of 213 dermatology-related applications were identified. Approximately 10.8% of the applications were related to skin diseases, 23.5% were related to skin care, and 65.7% were others. Among the applications related to skin diseases, 4.2% were self-surveillance/diagnosis apps, 3.3% were disease guide and reference apps, and 1.4% were dermoscopy apps. No applications were related to educational aid and photography search. Among the applications related to skin care, 12.7% were self-surveillance/diagnosis apps used for general skin care, 3.8% were skin care apps, and 2.8% were cosmetics apps. Other categories included shopping (24.9%), advertisements (15.0%), and hospital searches (15.0%). @*Conclusion@#The development and use of dermatology mobile applications can contribute to the efficient delivery of accurate information to patients, educate patients, and further reduce the prevalence of skin diseases. Therefore, dermatologists should play a leading role in presenting the direction for dermatology-related mobile application development.

ABSTRACT

Apocrine hidrocystoma, also called apocrine cystadenoma, is a benign cystic tumor-like lesion that arises from the proliferation of apocrine glands. Clinically, it usually occurs singly as a unilocular or multilocular, dome-shaped translucent cyst. Histologically, it appears as unilocular or multilocular cysts composed of an inner layer of single or double layer of secretory columnar epithelium with decapitation secretion lying above an outer myoepithelial cell layer. Apocrine hidrocystomas mostly occur within the head and neck region and involvement of genitalia is extremely rare. This paper emphasizes the importance of considering the differential diagnosis of a genital cystic lesion. Herein, we report a case of apocrine hidrocystoma occurring in the penis and compare the clinicopathological characteristics of apocrine hidrocystoma in genitalia with the previous cases.

ABSTRACT

Although the exact etiology of cutaneous angiosarcoma remains unclear, MYC gene amplification has been recently discovered as a new pathogenesis. MYC is a proto-oncogene, and aberration of MYC signaling in malignancies is associated with tumor metastasis, recurrence, and mortality. Moreover, upregulation of the miRNA polycistron, miR-17-92 cluster, were confirmed in both cutaneous angiosarcoma and multiple myeloma with MYC amplification. The correlation between MYC and miRNA expression is predictable as the coincident aberrant phenotype in two diseases. Moreover, the exploiting MYC dependency may be an attractive disease-specific strategy for the diagnosis and treatment of patients who are unaware of the causes of cutaneous angiosarcoma. Herein, a rare case of cutaneous angiosarcoma of the foot, which is also the first case of cutaneous angiosarcoma accompanied by multiple myeloma, has been described.

ABSTRACT

Elephantiasis nostras verrucosa (ENV) is an uncommon condition caused by repeated inflammation and lymphatic obstruction. It occurs mainly in the lower extremities and is characterized by skin changes, including hyperkeratotic mossy papules and plaques, non-pitting edema, and cobblestone-like appearances. ENV can be diagnosed based on its typical clinical manifestations (pseudoepitheliomatous hyperplasia with enlarged lymphatic spaces, fibrous tissue proliferation, and chronic inflammation) and by skin biopsy. Although ENV is difficult to treat, reduction of lymphedema and use of keratolytic agents may be helpful. To our knowledge, only three cases of ENV have been reported. However, the case of ENV treated with oral acitretin has not been reported in domestic literatures. Therefore, we report the case of a 45-year-old woman who developed ENV. She had a past history of cellulitis and was successfully treated with oral acitretin.

ABSTRACT

Allergic contact dermatitis is an inflammatory condition associated with periorbital erythema, edema, and pruritus. The periorbital skin is relatively thin compared with the skin over other facial areas; therefore, it is vulnerable to allergen penetration and may show a variety of cutaneous manifestations. Recently, vision enhancement surgery is a widely performed procedure, and the prevalence of senile cataract and glaucoma is increasing. The prevalence of periocular allergic contact dermatitis is increasing secondary to the growing use of topical ophthalmic medications. Several studies in Korea have reported periocular allergic contact dermatitis secondary to the use of topical ophthalmic medications including latanoprost (Latano®), fluorometholone (Tolon®), polymyxin B (Terramycin®), atropine sulfate (Atropine®), neomycin sulfate (Cambison®), and befunolol hydrochloride (Bentos®), among others. However, ofloxacin (Effexin®)-induced allergic contact dermatitis has not been reported in the domestic and/or foreign literature. We report a case of periocular allergic contact dermatitis secondary to the use of ofloxacin ophthalmic ointment.

ABSTRACT

Disseminated superficial actinic porokeratosis (DSAP) is a rare keratinization disorder. It is histopathologically characterized by the presence of coronoid lamellae and clinically by lesions showing central atrophy with elevated borders. Squamous cell carcinoma originating in the classical Mibelli subtype of porokeratosis is well-documented; however, few reports have described squamous cell carcinoma in DSAP lesions. We report the case of a 76-year-old woman with DSAP who developed Bowen's disease, actinic keratosis, and keratoacanthoma on her face.

ABSTRACT

Skin has long been known to be a very radiosensitive organ. Furthermore, the damaging effects of radiation exposure induced by fluoroscopic procedures have been recognized for some time. However, accurate diagnosis of fluoroscopy-induced chronic radiation dermatitis (FICRD) can be very challenging, since the provided clinical history regarding radiation exposure may not be accurate and the signs of cutaneous radiation injury may develop months to years after radiation exposure. Recognition of FICRD is important to avoid unnecessary treatment and interventions and also guarantees appropriate surveillance for radiation-induced malignancy. We herein report a case of a 65-year-old woman who was diagnosed with radiation dermatitis after endoscopic retrograde cholangiopancreatoscopy (ERCP).

ABSTRACT

BACKGROUND: New-onset refractory status epilepticus (NORSE) occurs in people without a history of seizures. In these cases, the seizure causes are unclear, and the seizures are not controlled by standard treatment. Autoimmune encephalitis (AIE) can be a cause of NORSE. Cryptogenic NORSE may be associated with AIE, but antibodies associated with the condition have not yet been identified. Primary immunotherapy may not be effective for AIE. Rituximab has improved the prognosis in some cases. CASE REPORT: We treated a cryptogenic NORSE patient with a combination of antiepileptic drugs and immunotherapy. On the 13th hospital day, the seizures were controlled, but the patient remained in a coma. The patient rapidly recovered after administration of rituximab started on the 26th hospital day. CONCLUSION: Rituximab may be helpful for cryptogenic NORSE patients in whom primary immunotherapy controls seizures, but fails to improve consciousness.
